One of the fastest ways to erode reporting culture is inconsistency. If similar incidents are handled differently depending on who is involved or how busy the operation is, credibility declines.
People watch for patterns. Are reports acted on? Are outcomes communicated? Is feedback provided? Consistent follow-through reinforces that reporting has purpose, not just paperwork.
NEAR-MISSES ARE CULTURAL INDICATORS
High near-miss reporting is sometimes misunderstood as a sign of poor performance. In reality, it often signals a mature culture. It indicates that people are noticing weak signals and feel safe enough to surface them.
Low reporting, by contrast, can reflect underexposure rather than low risk. An absence of data does not mean an absence of hazards.

Industrial organisations invest heavily in risk frameworks and digital reporting tools. These systems are essential, but they rely on human input. Culture determines whether that input is complete, delayed, or absent.
Strong reporting cultures normalise uncertainty and imperfection. They recognise that early reporting of small issues prevents larger events. The focus shifts from “Who is responsible?” to “What can we learn?”
